The Intake Services Coordinator takes the support requirements of each individual into consideration. Along with this the individuals are provided information packages. The corporate DVD is also available for viewing.

The Intake Process for prospective individuals is compromised of six basic components:

1. Information Inquiry
2. Service Overview
3. Intake Orientation
4. Determination of Eligibility
5. Assessment
6. Completion of an Individual Service Request for funding

Reporting

A monthly statistical form is completed to identify numbers of individuals requesting service.
